BCEN's credential for registered nurses who provide critical care during ground and air medical transport, validating the specialized knowledge needed to manage critically ill or injured patients in a mobile, resource-limited environment — distinct from bedside critical care nursing because transport nursing requires managing the full scope of critical care independently, often without immediate backup, while also accounting for the physiological effects of transport itself.

Who it's for

Registered nurses working on ground critical care transport teams, air medical (helicopter or fixed-wing) transport teams, and other inter-facility or scene-response transport roles. It's a natural next credential for critical care or emergency nurses moving into transport nursing, which combines both specialties and adds transport-specific physiology and logistics.

What's covered

  • Medical Emergencies in Transport (35%) — managing cardiovascular, respiratory, neurological, endocrine, and other medical emergencies during transport
  • Trauma Emergencies in Transport (30%) — managing traumatic injuries, including airway, hemorrhage control, and multi-system trauma, in a mobile care setting
  • Special Populations & Considerations (20%) — pediatric, obstetric, neonatal, bariatric, and geriatric considerations specific to transport nursing
  • Transport Operations, Safety & Professional Issues (15%) — flight/transport physiology, safety protocols, communication, legal and ethical issues specific to the transport environment

What to expect

The CTRN exam is a multiple-choice exam testing both core critical care and emergency nursing knowledge and the transport-specific complications that don't come up in a fixed hospital setting — the physiological effects of altitude and acceleration/deceleration forces, managing a deteriorating patient with limited equipment and personnel, and coordinating safely within a moving vehicle or aircraft.

How to prepare

Because CTRN candidates typically already have strong critical care or emergency nursing backgrounds, the highest-value study time is often the transport-specific content that a bedside nursing background doesn't cover — flight physiology, transport safety protocols, and the practical realities of managing an unstable patient with a two-person team and limited resources. Reviewing core medical and trauma emergency management is still essential, since the exam expects independent, high-level clinical judgment without assuming a full hospital care team is available.
