Know the Format Going In
The ACSM-EP exam has 140 multiple-choice items — 125 scored, 15 unscored pilot items you won't be able to identify — delivered over a 210-minute seat time. Since scored and unscored items look identical, treat every question with the same level of care rather than trying to guess which ones don't count toward your score.
Confirm Eligibility Well Before You Register
You need a bachelor's degree in exercise science, exercise physiology, or a closely related field from a CAAHEP-accredited program, plus current CPR/AED certification that includes a live practical skills component (not an online-only course). Confirm your program's accreditation status and your CPR/AED certification details weeks ahead of your target exam date — an eligibility gap discovered at registration time is an avoidable delay.
Prioritize Risk Stratification and Referral Judgment
A large share of the exam tests whether you can correctly sort a client's risk level and decide whether independent ACSM-EP programming is appropriate, or whether physician clearance or referral to a more clinically supervised setting is required. Practice scenario questions specifically built around this judgment call, and when you miss one, work through exactly which detail in the scenario should have triggered a different decision — that pattern recognition matters more than memorizing the risk categories in isolation.
Don't Neglect Management
At 15% of the exam, Management is easy to underweight in your prep because it doesn't feel like core exercise science. Staffing, risk management and emergency action plans, equipment procurement, budgeting basics, and program evaluation all appear. Because this content is largely straightforward once you've studied it, a modest, focused time investment here yields a strong return relative to more conceptually difficult domains.
Drill the Field Test Protocols
Know the Rockport Walk Test, YMCA cycle test, Åstrand-Rhyming test, Queens College Step Test, and Senior Fitness Test battery by name, including what population each is appropriate for and what's actually being measured. These specific protocols show up repeatedly in Health and Fitness Assessment scenario questions, and vague familiarity isn't enough — you need to recall the specific procedural details.
Separate the Behavior Change Theories Clearly
Exercise Counseling and Behavioral Strategies draws on several theories with overlapping vocabulary — Transtheoretical Model, Health Belief Model, Social Cognitive Theory, Theory of Planned Behavior, Self-Determination Theory — plus Motivational Interviewing technique. Build clear distinctions between them (e.g., self-efficacy belongs to Social Cognitive Theory, perceived behavioral control belongs to Theory of Planned Behavior) so scenario questions don't trip you up on theory attribution.
The Week Before
Review your weakest domain — for most candidates that's either risk stratification judgment calls or the Management content — confirm your testing appointment and ID requirements, and get a full night's sleep. The exam's mix of assessment recall, scenario-based judgment, and operational/management knowledge rewards being mentally sharp and well-rested more than last-minute cramming.
